one of 4 things going on 3 are lag the other is out and out cheating.

1) alot of albs causing your connection or system to lag behind.

2) the alb you trying to kill lagging badly(almost impossible to tell this from #4)

3) sever lag

4) the cheatin bastaige is using a lag exploit alot of folks use this it used to be window dragging but now there is several ways to do it, one is resizing your chat window, something like this he sticks to you the resizes chat raipidly and he will be to far away to target (I think this is to prevent easy killing of lag ghosts) then pop he is on you smacking you down (at this point you should beable to target and hit him.)

All that being said it seems you could not get any of them so I would side on you lag or server lag.
